Physical Therapist
Job Responsibilities:
Engage in and carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient’s progress toward achieving functional goals.
Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
Review functional needs of patients and adjust care plans to ensure optimal patient outcomes.
Design and implement a plan of care for patients based on a thorough assessment of potential development and incorporate participation by significant others whenever it seems appropriate and continuously conduct therapy practice within defined standards of care.
Direct the assistance of support staff to achieve patient’s functional goals.
Communicate effectively with other professional and support staff to achieve positive patient outcomes.
Document evaluations, treatment goals, and plans.
Regularly update documentation and maintain prescriptions and all required signatures.
Additional duties will include those within the realm of the PT field of knowledge and ability.
Qualifications:
Formal education that includes a degree in Physical Therapy with a minimum of one year of experience required.
Qualified candidates will have a current, unencumbered license in Physical Therapy.
Excellent patient and co-worker communication. Organizational, time management, and computer/EMR skills a must.
